Consumer Complaints
OMLA has generated a Consumer Complaint Form for use by individuals who wish to file complaints with the Oklahoma State Department of Health (O.S.D.H.) via the Code Enforcement Division. Our form for OMLA use only is:
The O.S.D.H. now has their complaint form on their web site and the direct link to their site is:
then click on the Applications Forms and Fees header. Go to the bottom of the new page and click on the Complaint Form header.

The following is true using either path to the form:
A dialog box will appear on the screen and ask if you want to open the file. Click on "open" and a new screen will appear with the complaint form. Maximize the screen if required.
When the State Complaint Form opens in .doc format, it can be saved or printed off. Complete the form and forward to the State per the instructions on the sheet.
