OMLA has generated a Consumer Complaint Form for use by individuals who wish to file complaints with the Oklahoma State Department of Health (O.S.D.H.) via the Code Enforcement Division. Our form for OMLA use only is:

When the State Complaint Form opens in PDF format, it can be saved or printed off. Complete the form and forward to the State per the instructions on the sheet.
